As we enjoy the beginning of Spring, I hope our second online newsletter finds everyone in good health and spirits. Winter was busy for this podiatrist, with two major skiing trips. No, I didn't compete in the Vancouver Olympics, but I did watch every night after skiing. My trips were to Beaver Creek, Colorado and Heavenly Valley, South Lake Tahoe. In the spirit of full disclosure, only trip # 2 was business related as I spoke at the Los Alamitos Physician Conference. I'm excited about this newsletter as we've included two PODCASTS, one on 'fitness shoes' and the other on a new piece of medical equipment called PADNET. We've also been updating my website blog on a regular basis with a number of timely articles. Please take a minute to read and post your comments. We appreciate any and all feedback.

Out of Options, Patient Turns to Dr. Vikki & MicroVas ~

Long Beach native Louise Kripal was getting nowhere with the treatment for a serious ulceration on her ankle. "I went to see countless doctors, including a vascular surgeon about my condition. There was no improvement. I went to a wound care center, which tried everything, but nothing seemed to help," Louise added. "I tried different medications. After no success, one doctor indicated a foot amputation was a distinct possibility."

Louise already suffers from severe rheumatoid arthritis. She gets around in a wheelchair and uses her left foot to transfer out of the chair, so amputation surgery wasn't an option. Her brother, Vincent, recommended Dr. Foley after hearing about MicroVas. "When I told my current doctor about MicroVas (www.MicroVas.com), he wasn't very optimistic."

"Over the last three years, we've had tremendous success using MircoVas Therapy," said Dr. Foley.

MicroVascular Therapy (MVT) works directly and mechanically to elevate blood flow through neuromuscular stimulation of the venous muscle pump. "We've used it successfully in the treatment of neuropathy, pain management, arthritis and wound healing. Louise's ulceration was serious. Combine that with other circulation issues and we had our work cut out for us." said Dr. Foley. "I was going to do an allograft, which is from human cadaver skin, but decided to see how Louise would respond to MircoVas first."

Dr. Foley scheduled Louise for three treatments per week. "Dr. Foley was fabulous. She was thorough and patient," says Louise. "She went above and beyond the call of duty." In the first month, Louise saw and felt an improvement. "Dr. Foley even changed the bandaging, which helped." In treatment for over eight months, Louise's ulcer is close to healing entirely. "I think over the next couple weeks it will be healed and I can cut back on the treatment. Another positive thing is that I no longer have to take any pain medication." SNUG CLEATS, REPEATED KICKING LEAD TO INGROWN TOENAILS

Soccer fields are filled with the future soccer stars of the sport this spring. And just like college or professional players, juveniles suffer their fair amount of injuries This is a bad time of year to be a juvenile toenail. Dr. Foley sees a lot of children with ingrown toenails during soccer season.

"It seems like every child is enrolled in a league and some, more than one," says Dr. Foley "The young kids wear hand-me-down cleats that don't fit exactly right. The older kids like tighter cleats to get a better feel for the ball and the field." A former collegiate player at UCLA, Dr. Foley had her share of injuries growing up.

"Ingrown toenails are such a small problem with such big pain. If you hit the corner of that affected toe, it shoots an intense pain that lingers." Unfortunately, many children don't tell parents about the problem because they're afraid to miss a game. "By the time they come to my office, a small problem has turned serious," she says. Young soccer players sidelined by an ingrown toenail may be able to get back into the game pain-free thanks to a simple, 10-minute surgical procedure.

During the short procedure, Dr. Foley numbs the toe and removes the offending portion of the nail. Various techniques can permanently remove part of a nail's root too, preventing it from growing back. Most children experience very little pain afterwards, and can resume normal activity the next day.

Parents should teach their children how to trim their toenails properly. Trim toenails in a fairly straight line, and don't cut them too short. "Keeping toenails trimmed correctly sounds like something so simple. You'd be amazed how many people, children as well as adults do it incorrectly," Dr. Foley says.

Parents have to make sure their children's cleats fit, since a child's shoe size can change within a single soccer season. If a child develops a painful ingrown toenail, soaking their foot in room-temperature water and gently massaging the side of the nail fold can reduce the inflammation.

Achilles Tendon Injuries

In the past month Achilles tendon injuries have been prominently discussed on the sports pages. World renown soccer star, David Beckham ruptured his Achilles tendon in early March and recently Laker center, Andrew Bynum strained his Achilles. If you recall, Olympic champion volleyball player, Misty May-Treanor tore her Achilles tendon, not on the sand, but on the TV-show, "Dancing With the Stars." A word to every athlete, whether you're pro or a weekend warrior- DON'T MESS WITH YOUR ACHILLES TENDON.
 
The Achilles tendon is the largest tendon in the body. It can become weak two inches above the heel due to a lack of blood supply. This is where most ruptures occur. May-Treanor and Beckham both experienced this level of injury. Sports requiring jumping, pivoting, and running put strain on the tendon.
 
The tendon becomes weaker with age and can literally rupture or snap. Full recovery is a painfully laborious process. I've found that athletes get 80 -90 percent of their strength back in the injured leg. David Beckham starts a water physical therapy program next month, six weeks after his recent surgery. He won't be able to play soccer for 6 months and will miss the World Cup.
 
For Laker fans, the news is much better. Andrew Bynum's strained Achilles should only sideline him to rest and rehab for 3 weeks and he should be ready for the play-offs. This isn't an injury you want to have happen. A good prevention program includes stretching, strengthening, wearing good shoes, and orthotics. If you feel consistent pain in this area, don't push it. Come in for an exam.

  Broccoli Salad

After we veered off the "healthy" road last newsletter with my mom’s immensely popular BEER BUTT CHICKEN, we're getting back to healthful eating this edition. Below is a wonderful recipe courtesy of the FOOD NETWORK's Paula Dean. This one only takes 10 minutes to prepare.

Enjoy and be healthy!

Ingredients
        1 head broccoli
          6 to 8 slices cooked bacon, crumbled
          1/2 cup chopped red onion
          1/2 cup raisins, optional
          8 ounces sharp Cheddar, cut into very small chunks
          1 cup mayonnaise
          1 tablespoon white wine vinegar
          2 tablespoons white vinegar
          1/4 cup sugar
          1/2 cup halved cherry tomatoes
          Salt and freshly ground black pepper

Directions
Trim off the large leaves from the broccoli stem. Remove the tough stalk at the end and wash broccoli head thoroughly. Cut the head into flowerets and the stem into bite-size pieces. Place in a large bowl. Add the crumbled bacon, onion, raisins if using, and cheese. In a small bowl, combine the remaining ingredients, stirring well. Add to broccoli mixture and toss gently.
